TEOSYAL RHA 4 – 23 mg/mL hyaluronic acid gel, 0.3% lidocaine
Teoxane manufactures TEOSYAL RHA 4 as a Class III device carrying CE marking. Its category is the dermal filler: volume is added where the gel sits, and the implant breaks down over time rather than remaining in tissue. Neighbouring products from the Teosyal line are held in the same warehouse.
Composition is not what defines this device. Concentration and crosslink density match RHA 2 exactly. Scope of licence is the difference: contouring, deep folds through thick skin, restored volume – and three regions ruled out that RHA 2 retains.
Reach for RHA 4 where projection is wanted across the mid or lower face, and where the gel will rest beneath the skin or on bone covering rather than inside the dermis.
Product overview
| How correction happens | A crosslinked hyaluronic acid gel adds volume where placed, then resorbs |
|---|---|
| Aesthetic indication as stated by Teoxane | Remodelling of facial contour; deep wrinkles and folds through thick skin; restored or augmented volume and correction of slight tissue descent across mid, perioral, lower and upper facial regions |
| Therapeutic indication as stated by Teoxane | Reconstructive surgery, filling of scar depressions, rebuilding volume lost to lipoatrophy in patients on HIV treatment |
| Where it goes | Within the dermis, beneath the skin, or on the periosteal surface, by site |
| When it shows | At once, since the effect is mechanical |
| How long Teoxane estimates it lasts | 15 through 18 months; skin type, site, quantity placed and patient physiology all bear on it |
Specification

| Regulatory position | Class III device, CE marked |
| Active | Hyaluronic acid at 23 mg/mL, non-animal in origin, mean molecular weight exceeding 1 million Dalton |
| Crosslinker | BDDE; crosslinking degree under 5% measured by NMR; BDDE residue under 1 ppm |
| Anaesthetic content | Lidocaine hydrochloride at 0.3% by mass |
| Buffer system | Phosphate, pH 7.3, made up to 1.2 mL |
| Keep at | 2 to 25 degrees Celsius, out of direct sun |
| Who may administer | Healthcare professionals trained and licensed under the law that applies to them |
Mechanism
Dermal hyaluronic acid content declines with age. This gel restores volume physically, occupying the plane into which it is placed, correcting lines and depressions until resorption completes. Across the Teoxane clinical study programme, aesthetic improvement of the treated site was recorded in 70 to 100 percent of patients; for the medical indications, patient-reported satisfaction stayed above 70 percent through six months from first injection.
Identical gel, inverted brief. One product corrects inside expressive skin; this one lifts from underneath.
Treatment areas
| Region | Sites listed by the manufacturer |
|---|---|
| Mid face | Nasolabial folds, cheeks, cheekbones |
| Around the mouth | Marionette lines |
| Lower face | Chin and jawline |
| Upper face | Temples |
Quantity per point and per appointment is governed by the instructions for use. Teoxane sets a ceiling of 6 mL in one session, drawn from preclinical and clinical data.
Browse by clinical objective: deep wrinkles; volume restoration; facial contouring.
Administration
Suppliers approved by Teoxane provide the pair of sterile 27G 1/2″ needles boxed with every syringe. A blunt 25G cannula is permitted in their place, tested against the TSK 25G/50 mm; an appropriately sized sharp needle creates the entry. Further cannulas and needles are held in stock. Teoxane advises keeping its gels clear of quaternary ammonium salts, benzalkonium chloride among them. Massage follows placement so the material spreads evenly, and the plunger is released early enough that nothing tracks back out of the channel.
Safety profile
Pain, tenderness, erythema, bruising, swelling, induration, papules and itching occur at the site in 1 patient per 100 or more often; these are mild as a rule and settle unaided. Discoloration, inflammatory response, infection, short-lived numbness or paraesthesia, allergy, nodules and implant migration sit in the band from one per thousand up to one per hundred.
Placement inside a vessel or beside one carries the risk of embolisation, occlusion, ischaemia and necrosis of skin or tissue. Teoxane records these as extremely rare, while noting that the gravest occlusions have caused sight loss, temporary or lasting, and cerebral ischaemia or bleeding leading to stroke and, in some reports, death. Injection at the nose or between the brows raises that risk. Blanching, mottling, pain or numbness at any stage means the injection stops.
Lidocaine is present, and can return a positive doping result.
Contraindications
- Skin disorder, inflammation or infection at the site or near it
- Known hyaluronic acid hypersensitivity, or a history of severe allergy or anaphylaxis
- Autoimmune disease
- Pregnancy, breastfeeding, or age under 18
- Known hypersensitivity to lidocaine or to amide anaesthetics
- Cardiac disease, or beta blocker therapy for it
- Porphyria, or epilepsy
- Hepatocellular insufficiency, or any therapy directed at liver disease
Teoxane also rules out the region around the eye, taking in eyelid, tear trough and lateral canthal lines; the area between the brows; and lip tissue. Excluded as well: the epidermis, the inside or vicinity of vessels and nerves, and any depth outside the dosage section. Combination with peels, laser or ultrasound is excluded too. Nothing here substitutes for the instructions supplied with the device.
Choosing within the range
Two devices, one formulation, and only the label between them. RHA 2 holds the upper face, the eye region, mid face and the area around the mouth, lips included. RHA 4 gives up the first two and the lips, and gains the periosteal plane in exchange. Correction inside mobile skin points to the first; lift from below points to this one.
Frequently asked questions
RHA 4 or RHA 2 for a given case?
Composition will not decide it. Both sit at 23 mg/mL with crosslink density under 5 percent. The label decides: RHA 4 covers contour remodelling and volume, reaches the periosteal surface, and rules out the eye region, the frown area between the brows and the lips. RHA 2 keeps all three.
What planes appear on the label?
Three: within the dermis, beneath the skin, or resting on the periosteal surface, chosen by treatment site.
What does the manufacturer rule out?
Injection around the eye, covering eyelid, tear trough and lateral canthal lines; the area between the brows; and lip tissue.
What longevity does Teoxane publish?
An estimate running from 15 through 18 months. Skin type, the wrinkle addressed, injection site, quantity placed, physiology and lifestyle all shift that figure.
